1. Introduction and Purpose of the Study: MoCo Pride Center wants to hear from YOU about your experience with our building, staff, and programs to make sure that we are providing the best for our community! All questions are optional unless otherwise marked. Your responses will be used to guide future program direction at MoCo Pride Center, assess efficacy of ongoing programs, and determine budget priorities for County leadership.
2. Description of the Research: The Community Needs Assessment Survey has three sections — Program Feedback, about specific MoCo Pride Center programs, General Feedback, about your sense of Montgomery County and the Pride Center as a whole, and Demographics, about who you are. You do not need to fill out every section of the survey to submit a response. You can skip any questions you do not want to answer unless they are marked as “required.”
3. Potential Risks and Discomforts: The information you submit via this survey will be used to generate official demographics reports submitted to the Montgomery County Council on a monthly basis. Optional identifying information like your name will NOT be included in any such reporting, but will be visible to Pride Center leadership. Your personal information may be used to contact you about services that fill your expressed need, especially if the need is urgent and significant.
4. Potential Benefits: This survey is a direct line to MoCo Pride Center leadership for participants to ensure their experiences are being heard and their needs are being met. Participants will be taking an active role in shaping program direction at MoCo Pride Center by letting us know how we can best serve the community. You may also be passed service recommendations based on your responses, ensuring that you receive the care you deserve.
5. Confidentiality: No names or other identifying information will be used when discussing or reporting data. Data will be aggregated to meet Montgomery County reporting obligations. Results of this survey will only be shared with the MoCo Pride Center Program Director, CEO, and Montgomery County Council. By clicking “I agree” above, you authorize the use and disclosure of your survey responses for analysis and presentation.
6. Compensation: Participants will not be compensated for participation in this survey.
7. Voluntary Participation and Authorization: Your decision to participate in this survey is complete voluntary. If you decide to not participate in this study, it will not affect the care, services, or benefits to which you are entitled from MoCo Pride Center or Montgomery County.
8. Withdrawal of Authorization: You can withdraw from the survey at any point if you choose not to continue. If you choose to participate in this survey, you may withdraw your responses at any time without penalty. Send an email to the team at hello@mocopridecenter.org requesting your input be removed, and your responses will be permanently deleted.
9. Cost/Reimbursements: There is no cost for participating in this survey.
